only guitar will not be provided so please kindly bring your own guitar.

Audition Fee : RM5 per person.

Audience Fee : RM5 per person.

P/s. non of the money collected will be going into the committee's pocket,it's for covering up the cost of the place & to raise fund for prom.

Questions & Answers regarding Prom :)

WHY SHOULD YOU ATTEND PROM?

A lot of effort has been made to organize this event for everyone,especially for the seniors of twealvean's.It’s not only about the quality food,classy ballroom and dressing up. It is a once in a lifetime event that is going to give us a  wonderful memory & also a perfect finale ending to our high school life. We really want to make this prom a success,So please do not hesitate because YOUR PRESENCE MAKE A BIG DIFFERENCE! :)

HOW TO MAKE RESERVATION?

• Reservation for A table can be made by handing in 10 names or a minimum of 5 names first. (Add in the rest later)
• When payment is not made, reservation is not confirmed. If there are other students interested in purchasing the table, the committee reserves the right to open and sell the table to others who paid first.

HOW TO CONFIRM RESERVATION?

• You can confirm A table when minimum 5 students have paid the ticket price (RM90 early bird/RM110 normal ticket), which means the amount paid must be at least 50% of each ticket per person or 50% of each table as a deposit. 
• The remain-dance can be paid before the 30 JUNE 2010 for early birds & 30 OCTOBER 2010 for the normal ticket.

HOW DO MAKE INDIVIDUALS RESERVATION?

• Email or look for the committees and hand in your details.(Name,Number,Class)
• All Payment should be made before dateline.
• For students who are interested to attend Prom(doesn't matter even if you're unsure as long as you are interested). Do let the committees know,so we can estimate the amount of people who are mostlikely attending.

Ticket reservations is opened. And the ticket prices are set as followed :

Early Birds.

Per head : RM 90.

Per couple : RM 160.

Normal Price.

Per head : RM 110.

Per couple : RM 200.

There will be limited places available for the Early Bird tickets,so do proceed your payment before the following date 30 JUNE 2011. However, if the limited places set for the Early Bird tickets are filled before the due date, reservation for Early Bird tickets will be closed. And remember, Form 5s from SMK USJ 12 have the first priorities. However,The due date for the Normal tickets will be on the 30 OCTOBER 2011.

(Terms and Conditions)

• Upon purchasing/redeeming your tickets, provide your contact details (i.e. handphone number, and full name) on a piece of paper and hand it to the ticket seller/or email us.
• Tickets purchased are nontransferable.
• Tickets purchased are not refundable unless complications occur.
• There will be a numbered arrangement; seats are limited.
• Tickets are coded with seat numbers and are not subjected to last minute changes
• A total of 10 seats per table are available. The management comittee recommends, to avoid seated with strangers and ineluctable awkward hours, booking/purchase a table in whole is adviced.
• You may request for a specified seat. The management comittee however is not responsible for any undesirable events.
• Tickets and seats sold are subject to change without prior notice.
• The management committee under any circumstances is not held liable for losses during the dinner.
• The management committee exhorts for punctuality in ticket collection.
• Couple tickets will only be sold to BGRs(Boy Girl Relationships). 
• All ticket sales are final and people who are evicted will not be refunded.

Rules & Regulations

The committee of Glamour & Glitz would like to inform guests that:

Dress Code:
Strictly formal

Strictly no jeans allowed. People wearing jeans will not be granted entry into the ballroom
Strictly no tee-shirts allowed. People wearing tee-shirts, similarly, will not be granted entry too.

• It is a prom, not a party - no liquor is allowed.
• It is a prom, not a party - it's a non-smoking event.
• It is a prom, not a party - there are protocols and regulations which have to be followed, no causing any form of chaos, havoc, or trouble.

Age Requirement

If there is any questions regarding the age limit of people who wish to attend this prom, I hereby dedicate a whole post to it just for clarification purposes.

As this is a Prom conducted by us to cater for the Form 5s last year in high school, priority will be first given to the Form 5s students which include the early bird tickets.

Minimal age requirement is 15. These 15 year olds will be granted access provided they purchased a ticket. However, if people younger than age 15 have dates who meet the age requirement, they will be granted entrance(provided they purchase a ticket and attained permission from the committee for their date).

Maximum age requirement is 17. However, if people older than the age 17 or are no longer in school have dates who meet the age requirement, they will be granted entrance(provided if they purchase a ticket and attained permission from the committee for their date).

Any single person(s) who do not meet the age requirements will not be granted entrance and will not be allowed to purchase tickets. Any couple who does not meet the age requirements will not be granted entrance as well.

Any personnel who exceeds or has not achieved the age requirement and has not obtained permission from the committee will not be granted entry into the ballroom.
